"This study demonstrates that early feeding of tummy tuck patients may prevent the need for postoperative hospital admission for intravenous hydration-at least in those patients who can maintain adequate hydration with oral intake alone-and allow for faster hospital discharge," said Alan Matarasso, MD, a board-certified plastic surgeon in New York, NY, lead author of the study.

While there may be other reasons for admission, routine postoperative admission to prevent dehydration may not be necessary. These findings could change the way we care for our postoperative abdominoplasty patients, improving their comfort and safety, and saving on health care costs.

Facelift surgery is a considerable investment of emotion and money which usually produces a return that is well worth it. A common question that I frequently hear in my Indianapolis plastic surgery practice is....how long will my facelift results last? This is a very understandable question and one that deserves a good answer.

A facelift operation treats the symptoms of the problem not the actual root cause of aging. (which no one knows how to stop) As a result, every patient will outlive the results of the surgery. I would argue that is the goal of every facelift patient...to live long enough to outlast the benefits of the surgery. While somewhat humorous, this speaks to the fact that facial aging is a continuous and ongoing process that can not be stopped. Therefore, no one should expect facelift results to be permanent or continue unaffected in the years ahead.

The classic facelift longevity answer is that it may last up to ten years. In many cases, the benefits of a tighter neck and jowls may well still be present this far out from the initial surgery. But there will be changes, a gradual relaxation of the neck and jowl skin. A gradual return of some sagging and loose neck and jowl skin will happen in all patients. This relaxation rate differs amongst patients and it can not be predicted whom will sag faster or slower. Thick heavy skin tends to sag quicker, while thin skin tends to hold up better.

The concept of skin relaxation is important to understand. Often referred to as 'rebound relaxation', the very tight neck and jowl area that one has right after surgery will relax somewhat over the first year. Laser liposuction, otherwise known as laser lipolysis, laser-assisted lipolysis or Smartlipo, is a new method of liposuction using laser technology. Fat is a unique tissue in that it is really globs of lipid (triglycerides) surrounded and encased in strands of connective tissue. To no surprise, fat has a fair amount of water in it which is very relevant in laser liposuction but largely unimportant in traditional liposuction. Fat is roughly 25% water. Running through the fat are small blood vessels and lymphatic channels. Some fat areas have globules that are quite big ('buttery fat") and other fat areas have smaller globules with a lot of connective tissue. ('gritty fat') Gritty fat, such as in the upper abdomen or back, takes a lot more force to break it up and be suctioned out.

The laser energy delivered to the fat via fiberoptic cables or probes is designed to raise the temperature of the lipids to above 42 degrees C. At this temperature, the fat cells are damaged and they release their lipids. (lipid liberation!) This liquid is easier to suction out that clumps of fat. The heat generated next to the tip of the cables causes coagulation of any blood vessels and lymphatic channels. A secondary benefit of this heat is its effect on the strands of connective tissue and the overlying skin. This coagulation of collagen tissue causes a beneficial shrinkage of tissue and, to some degree, the overlying skin.

There is currently much debate as to the 'ideal' laser wavelength for which to use to create this fat melting effect. Most of laser technology is based on matching the wavelength to the chromophobe (color) of the target tissue. Fat is a weak target because its yellow color, being lighter, is not a great absorber of wavelength energy. (same problem with the yellow and lighter colors in tattoo removal) Some manufacturers use 1064nm while others use 924nm which is theoretically more specific for lipid absorption. Both cause substantial heat generation and the general effect of heat transmission to the fat cell is probably achieved equally by both. Dual laser wavelengths today have become the new standard in an effort to target the water in the dermis (skin) as well. These include wavelengths such as 975 and 1320nms.

The sophistication of laser-assisted lipolysis is rapidly expanding. Dual wavelength energy delivery, monitoring internal temperature levels, and greater surgical finesse is rapidly changing the way liposuction is being performed. Better results and less recovery appear to be more than just marketing hype with this new laser-assisted liposuction technology.

Possible problems with breast reduction

The patient will be required to stay in the hospital for 1 day after the surgery. The possible problems associated with the procedure are:
-lactation ability is lost on most patients. Ability to feed is the exception in most cases
-sensibility of breast tissue will take almost 6 months to recover
-chances of asymmetry are there
-in large reductions, some tissue loss is possible, however this does not majorly effect the final result
-like any procedure, there may be chances of seroma or a hematoma formation

Results

Breast reduction is an extremely satisfying procedure, because the results are predictable to a large extent in the preoperative period. So it is important that a potential client look through the profile of their treating physician and asses really whether that person is actually capable of delivering all that he promises. This has the benefit that I never face the risk of fluid overload ( potentially dangerous condition, where there is too much water in the body, and the lungs get filled with water). The other benefit is that, what is aspirated is pure fat, and not water mixed with fat. This gives an exact estimate of the amount of fat taken out. We surgeons have a bad habit of including the amount of fluid aspirated into the total volume taken out also, so that we can boast of having done large liposuctions. What is important is not the amount taken out, but how much of actual fat there is.

The procedure is done with small incisions over selected areas of the body, after first discussing with the patient, and planning the site of the incision. These incisions generally are not visible and heal very well. They are only about ½ cm in length. There may not be a need to admit the patient in the hospital, so that the patient can go home the same day.

Precautions

After a liposuction, there is a need to wear specially designed pressure garment, over the areas liposuctioned to allow better skin redraping. This may take up to 6 weeks. Any anticoagulant use such as aspirin should be stopped 1 week prior to surgery; smoking should also be stopped 2 weeks before to 2 weeks after surgery for best results.

Newer technology

Power liposuction, vibro-liposuction, VASER, and laser liposuction are some of the newer innovations in this field.
In Power and Vibro Liposuction the mechanical effort to carry out liposuction is reduced by the use of special machines, which produce a to and fro rapid motion, thus reducing mechanical fatigue and making the procedure faster. However these are significantly more expensive than conventional liposuction.

VASER is the use of internal ultrasound device to melt fat. The advantage is that this is significantly faster, and allows the surgeon to work closer to the skin, and thus better skin draping is achieved, and thus better figure control is achieved. However this is a very expensive device, and expects the surgical cost to double if you ask for a VASER.
Laser lipo allows very good redraping action on the skin; however its role in actual fat reduction is a bit controversial. Thus this has not found use in general practice.

Liposuction Surgery - How Long Will it Last?

One of the very common and good questions most patients ask during a consultation about liposuction is the longevity of the results. How long will my results last? When there is pain, recovery and expense involved, the long-term benefits are extremely important. This is always a great question but the answer to it is somewhat complex.

My first comment about the longevity of liposuction comes from a two-year outcome evaluation of liposuction surgery published in a major plastic surgery journal. The results were disappointing but not surprising. Nearly half of the patients treated had gained weight in the liposuction-treated areas, some even bigger than they were before. (the good news is that half still had good results!) These results do not suggest that liposuction is not a good procedure or that it is not worth it. Rather, it strongly indicates that liposuction alone is not the entire solution to all of your body contouring needs....anymore than the lawn mower is worthless because the grass grows back. The long-term results are only as good as the continued effort you make through other non-surgical body shaping methods, such as proper nutrition and exercise. Liposuction is but one part of your overall body improvement program. If you don't change what got you there, you will eventually be back to where you started.

This being said....my basic answer to the question of whether the fat will return after liposuction is...if your weight is stable, then the results will be fairly long-term. However, that answer must be tempered by the location of where the fat was removed. Not all fat is metabolically or structurally the same throughout the body. Fat even has a different appearance in different areas of the body. It stands to reason, therefore, that some fat areas function differently and not all fat areas are storage sites for excess calories exclusively. Clearly, for example, the stomach and waistline (flanks) is a preferential depository site. This is where almost everyone gains weight first.

Conversely, body areas such as the knees, back, and arms do not get big first and only with a lot of weight gain. These areas do not go up or down as fast as the stomach. Removal of 'non-depository' sites will maintain the results much longer as this is not the first place fat will go in most people. This depot site phenomenon is well recognized between the sexes. The buttocks and thighs is much more of a depot site for women but is not for men at all. (how many heavy men with large stomachs have a small butt....very common!) So these factors must be considered as well. Meaning, greater 'permanency' will occur from liposuction in areas that are not depot sites. Depot sites are more likely to regain weight than non-depot sites.

One myth of liposuction is that when one gains weight after surgery the fat will go to other non-liposuctioned areas. This is generally not true. There are still plenty of fat cells where liposuction has been, particularly in the stomach area. These remaining fat cells are fully capable of getting big again.

The world of dentistry is responsible for the evolution of teeth. When the anthropologists study these 3 humans, their findings would be remarkable. Pretend all 3 had braces during their lifetimes. This would be a reasonable assumption, since about 65% of people in the United States have worn braces at some point. I imagine the researchers would be impressed with the 3:3 ratios of straight teeth.

Let's say one of the humans wore veneers. I predict this would be an extremely important finding, because I doubt porcelain shells will still be around in the year 3031. This would be a feature exclusive to our generation.

Future anthropologists would find a whole world of culture within our teeth. Cavities, crowns, teeth whitening, sealants, missing teeth, gum disease and more would all be indicators of our culture. We have no way of knowing how anthropologists might analyze us, but it is certain they would make assumptions on the importance of appearance and upkeep of our teeth.

Now that 2008 is in the books, it would be interesting to look at the trends and statistics in cosmetic surgery and see what has changed...or what is the same. Unfortunately, we will have to wait until the summer when the surveys and statistics for last year are available. However, the numbers from 2007 have been in since last summer and a review of these and the impact from the economic downturn in the U.S. can make for some predictions as to what happened in cosmetic surgery in 2008.

According to the 2007 statistics on cosmetic plastic surgery from the American Society of Aesthetic Plastic Surgery, roughly 12 million cosmetic surgical and non-surgical procedures were performed in the U.S. with expenditures of over $13 billion. I suspect we will see in 2008 that these numbers are roughly the same and perhaps even down a bit. The last quarter of the year saw a dramatic dwindling of patients undergoing procedures although this may have been tempered by the peak of the 'holiday cosmetic season' when many plastic surgeons post their highest monthly averages in December due to being being off work and those patients having procedures may have already saved or budgeted for them at that point. While 2007 saw an 8% increase in procedures compared to 2006, 2008 will likely show zero growth.

Cosmetic surgery accounted for 2/3rds ($8 billion) of the expenditures compared to non-surgical procedures (e.g., Botox) but these dollar figures highly skew the number of procedures performed. Because each surgery costs considering more than any non-surgical procedure (often 10 - 20 times more), cosmetic surgical procedures pale in comparison to non-surgical procedures in terms of numbers, roughly 1.5 million (surgery) to over 10 million (non-surgery) procedures. This dramatic trend of increasing non-surgical procedures will likely continue to be seen from 2008, representing by far the largest growth area of cosmetic procedures.

The top five surgical procedures in 2007 were liposuction, breast augmentation, eyelid surgery (blepharoplasty), abdominoplasty (tummy tuck), and breast reduction.
